As an affiliate of Silver Sneakers, Renew Active, Prime, Silver&Fit and Active&Fit, eligible individuals simply pres- ent their healthcare fitness ID number and a valid driver's license or state ID to enroll in a no cost Fitness Member- ship at 4500 Fitness. RENEWACTIVE In a partnership between 4500 Fitness and Healthy Contributions, Renew Active is a fitness membership benefit for those who qualify through United Healthcare, or AARP-branded Medicare plans. Renew Active allows a fitness membership at no cost to the member. To see if you qualify, please contact your insurance provider. Contact your insurance carrier to see if you qualify for this benefit. SILVER&FIT Silver&Fit is designed to help older adults achieve better health through exercise and health education. Silver&Fit provides eligible members with no-cost or low-cost fitness memberships through arrangements with certain health plans. ACTIVE&FIT Active&Fit is designed to help the younger crowd achieve health through exercise. The Active&Fit program is a flexible, low-cost fitness membership available through your employer, health plan, or other participating organizations.

INDOORTRACK The indoor track is open to the community and fitness members. The track is included in a fitness membership. Non-fitness members may purchase a track pass allowing access to the indoor track only. Track passes expire 1-year from date of purchase. Proof of residency is required. Track passes are non-refundable or transferable. Resident: $5 | Non-resident: $75 Senior (62+) Non-resident: $50 WORK INDOWNERS GROVE? If you work at a business that is within Park District boundaries, you can receive resident rates on an annual Fitness Membership. Proof of full-time employment is required.
